Request a Report

To request a copy of a police report please fill out the PDF form or online form below.

Completed forms may be emailed, submitted online, mailed, faxed or delivered in person (our address and fax number are on the PDF form).

Report requests will be answered within 10 business days. You will receive an email (if provided) or phone call when your request has been completed. You must pick up your requested report from the Crystal Police Department.
